Field: Medicine and Life Sciences The technology of this project is a key technology and core content in the series of research and development projects for the intervention repair of diabetic peripheral vascular disease (DPVD), which has been successfully industrialized and upgraded, widely applied in the early intervention, diagnosis, treatment, and follow-up of DPVD; it is an integrated and innovative technology based on intervention repair. It pioneered the 'five-point method skin temperature' evaluation and follow-up system, providing technical standards and equipment for clinical diagnosis and treatment; it involves multiple disciplines such as clinical and preventive medicine, biology, systems and software engineering, and is the result of interdisciplinary integration and creation. About 75% of diabetic patients ultimately die from vascular-related complications. In response to the increasing incidence of DPVD, the rising mortality/morbidity rates, and the poor prognosis, Focusing on the significant challenges surrounding the acceleration of DPVD rehabilitation, the team has long explored the foundation, clinical, and translational aspects of interventional comprehensive treatment with the core aim of improving quality of life and overall survival (OS). Clear theoretical foundations closely related to interventional therapy have been established, a brand-new efficacy evaluation and follow-up system has been developed, and a series of interventional diagnosis and treatment devices and software have been created for intervention and management of all stages of DPVD. Technical Invention 1: The invention of new peripheral interventional repair technology. Addressing the difficulties in repairing damaged tissues due to DPVD, slow recovery, and the characteristics of multi-branch, multi-segment, and diffuse lesions, individualized targeted treatment, nursing, and rehabilitation plans have been formulated, including: interventional reconstruction of blood supply to the affected limb, series of stem cell transplants to improve microcirculation in the lower limbs, and patented tissue repair technology to promote ulcer healing. A total of 5430 patients from all stages were treated, with an ulcer healing rate of 87.4% and a limb amputation rate of 2.4%. The mortality rate is 0.3%, significantly better than the results of the latest relevant domestic and international research reports. Technical Invention Two: Creation of a Peripheral Microcirculation Quantification Diagnosis and Treatment System. In response to the lack of an effective, simple, and rapid evaluation system for DPVD, Tongji University's 'Five-Point Method Skin Temperature' therapeutic effect evaluation follow-up system was first proposed; based on patient vascular anatomy + function detection + effective microcirculation criteria, it effectively solves the problems of early screening and clinical follow-up. Multicenter randomized clinical studies have shown that the concordance rates of this system with vascular ultrasound and DSA are 98.5% and 97.8%, respectively, with an overall efficacy rate of 93.7%. On this basis, specialized oxygen saturation meters for toes and peripheral quantification microcirculation diagnosis and treatment systems were invented and have been widely used in clinical diagnosis and treatment. Technical Invention Three: Development of a series of specialized interventional new products. In response to the lack of specialized and effective treatment devices for DPVD, the team has developed a series of new products, including: directional valve medical treatment catheters, endothelial progenitor cell directional release system, drug-loaded small artery dedicated stents, flow reduction catheters for perfusion re-injury in peripheral vascular diseases, etc., which have been widely used in clinical treatment and trials. In the past three years, domestic sales have reached 367.3036 million yuan, generating foreign exchange earnings of 15.7380 million US dollars. Intellectual property rights: The achievements have obtained 15 national invention patents, 9 utility models, with 32 applications and substantive reviews; 5 certificates for proprietary intellectual property medical devices. Published 251 papers in SCI and EI journals, with an IF of 955.984, cited 2545 times. Published "Clinical Vascular Intervention Therapy"; formulated the 5th edition of China's clinical operation guidelines for DPVD, obtained 1 set of epidemiological survey software. Application and promotion: Approved as the National Clinical Research Center for Radiation and Treatment, established the Tongji University Diabetes Vascular Disease Diagnosis and Treatment Research Center, formed 6 regional branch centers, and has been clinically applied in 67 tertiary-level hospitals across the country. Over ten thousand patients have benefited from this, making significant contributions to the prevention and treatment of China's DPVD. This achievement has accumulated 2 national major science and technology plans, 25 NSFCs, etc.; trained 34 doctoral students and 60 master's students; received 15 provincial-level talent plans.
