Sudden Cardiac Death (SCD) refers to unexpected sudden death caused by various cardiac diseases. The main causes include malignant arrhythmias and acute myocardial infarction. How to achieve early warning and intervention has become the key to reducing the incidence of SCD. However, due to reasons such as uneven distribution of medical resources and relatively lagging informatization construction in China, the level of early warning and treatment for SCD in our country is still significantly insufficient. To solve the problem of inadequate treatment for SCD in China, Tongji University Affiliated Tenth People's Hospital innovatively established a risk warning assessment system for SCD through the North Shanghai population study, combined it with information technology platforms and the latest treatment plans, and systematically completed a new treatment model for SCD from early warning, transfer to treatment and follow-up. Provided new ideas and solutions for the prevention and treatment of sudden cardiac death in our country. The main innovative achievements are as follows: First, we established a risk assessment system for sudden cardiac death. We conducted large-scale population epidemiological surveys to comprehensively assess cardiovascular risk factors and developed the first risk assessment model targeting sudden cardiac death in China. This system can be implanted into an app that automatically distinguishes the degree of myocardial infarction risk in patients, namely low-risk, moderate-risk, and high-risk, thereby providing corresponding intervention measures for patients with different risks to reduce the incidence of myocardial infarction. This research achievement was cited in the 2017 European Society of Cardiology Guidelines on Vascular Diseases. Second, we developed an early warning device for sudden cardiac death. We developed and applied instantaneous electrocardiogram recorders (5312 cases), long-term 12-lead electrocardiogram recorders (2398 cases), and wearable 12-lead electrocardiogram recording vests (354 cases) for patients at low, moderate, and high risk of myocardial infarction. Combining the project team's independently developed artificial intelligence electrocardiogram automatic diagnosis system, the sensitivity of myocardial infarction electrocardiogram diagnosis system can be increased from 50% to 90%, and specificity from 85% to 94%. By collaborating with three Nobel laureates to establish an artificial intelligence algorithm for electrocardiogram signals, timely warnings are issued, reducing the average call-for-help time for myocardial infarction patients from 360 minutes to within 60 minutes, and the door-to-balloon time is also reduced from the original 140 minutes to 81 minutes. The project has won the Second Prize of Chinese Medical Science and Technology, with four national invention patents related products obtained, and has been transformed into a high-tech enterprise, valued at nearly 200 million yuan. Third, for precise treatment of sudden cardiac death, we have created a closed-loop medical service from early warning to treatment. We have developed an intelligent ambulance for sudden cardiac death, upgraded the digital equipment in the catheterization room, and established a globally first regional treatment network based on mobile PACS equipment. At the same time, a sequential therapy for cardiac rehabilitation is being implemented. The aforementioned equipment has constructed a closed-loop treatment system for sudden cardiac death. Through the implementation and promotion of the above sudden cardiac death prevention and treatment plan, the incidence of sudden cardiac death in the region has significantly decreased. The related report was published as the principal investigator (PI) in the New England Journal of Medicine in 2015. The aforementioned achievements have successively received funding from national key research and development projects, Shanghai's three-year action plan, and major projects supported by the Shanghai Science and Technology Commission, with 94 SCI papers published. Among them, there are 20 representative papers with a total impact factor of 82.146 points, cited 537 times, including 493 times by others. The related software and hardware equipment have been promoted and applied in 12 tertiary hospitals nationwide, with a cumulative diagnosis and treatment of 25,292 myocardial infarction patients, reducing mortality rates and improving the quality of life of patients.
