"Shi's Orthopedics" is a famous traditional Chinese medicine (TCM) school in China, which has made historic contributions to the modernization of TCM orthopedics and the establishment of higher education systems. It has a profound accumulation in the diagnosis and treatment of musculoskeletal injuries. Osteoarthritis and osteoporosis have become one of the main chronic diseases that affect the health of the elderly in our country, showing an increasing trend with the aging of society; ankle sprains are the most common sports injuries, accounting for about 40% of all sports injuries. While inheriting academic knowledge, the project team focused on tackling these bone and joint diseases that severely impact the national health level, exploring common prevention and treatment principles, and constructing a technical system for diagnosing and treating musculoskeletal injuries, achieving the following innovative results: (1) Organizing "Shi's Orthopedics" literature and combining it with the progress of modern clinical practice, Propose the general principles for the diagnosis and treatment of musculoskeletal diseases, which include 'emphasizing both qi and blood, with qi as the main focus; considering concurrent pathogenic factors such as wind, cold, phlegm, and dampness; recognizing injuries in treating people, and regulating the whole body; valuing both bones and muscles, ensuring bone alignment and muscle flexibility.' (II) Through clinical epidemiological research that combines 'disease, syndrome, and symptom,' it has been confirmed at the local syndrome differentiation level that the degree of pain in knee osteoarthritis is related to the differentiated syndromes, and the distribution of tender points on the knee is associated with anatomical structures. At the overall syndrome differentiation level, by combining tongue image parameters with local pathological elements of the joint, a correlation pattern between the tongue image in the middle root area and the cold-damp obstruction type of osteoarthritis was discovered. A new method for imaging assessment of knee pain locations, CKPM (Chinese Knee Pain Map), was created. Through RCT clinical trials, The effectiveness and safety of the Shi's formulation for anti-osteophyte synergy and membrane toughness plaster were verified; a 'Shi's Orthopedics Knee Osteoarthritis Special Treatment Plan' was formulated and its clinical superiority compared to traditional methods was validated. (III) Based on Professor Shi Yinyu's 'Kidney Tonifying Formula', a new drug for osteoporosis, 'Qi Gu Capsule', was collaboratively developed and obtained a new drug certificate (National Medicine Certificate No. Z20090039). In vivo experiments found that the Kidney Tonifying Formula could improve bone density and bone mechanical properties in osteoporosis animals, and inhibit bone resorption, with the kidney tonifying components significantly promoting bone formation compared to the kidney yin nourishing components; in vitro studies revealed that Astragalus membranaceus, Epimedium sagittatum, and Dendrobium officinale in the formula all have osteoblastic differentiation-promoting effects; It has been confirmed that icariin from the effective part of Epimedium can promote the proliferation and osteogenic function of bone marrow stromal stem cells and osteoblasts, which is one of the material bases for its efficacy. (IV) Under the guidance of Professor Shi Yin Yu's viewpoint 'For injuries to tendons and muscles, it is often appropriate to use both acupuncture and medication, with manual therapy as a secondary option', through clinical trials, the efficacy of traditional therapies such as fumigation, application, manual manipulation, and combination of acupuncture and medication in Shi's orthopedics department for acute ankle sprains (early and middle stages) and chronic ankle sprains was verified item by item. A cumulative study of 1077 cases confirmed that external treatment methods of Shi's orthopedics department can improve the pain and functional impairment of patients with ankle sprains, and have good safety, forming a new therapeutic theory for ankle sprains that emphasizes the importance of combining acupuncture, medication, and manual therapy, and treating them according to different stages. 127 papers have been published. Among them, 18 papers were included in SCI and cited 1113 times. Obtained 2 patents, trained 75 master's students and 19 doctoral students. Held the national continuing education class on 'Shi's Orthopedics Diagnosis and Treatment Technology' for 11 consecutive years, training 2172 trainees and receiving 265 postgraduate doctors from all over the country. Edited 7 national-level textbooks including 'Integrated Traditional Chinese and Western Medicine Bone and Joint Science'. Established the WeChat public account of 'Shi's Orthopedics', with popular science articles receiving 33,098 clicks. QiGu Capsules were included in the 'National Basic Medical Insurance Drug Catalogue (2017 Edition)' and selected as part of the 'Expert Consensus on the Prevention and Treatment of Primary Osteoporosis with Traditional Chinese Medicine (2015)'. The added value in the past 3 years has exceeded The project team is the leading unit of the National Administration of Traditional Chinese Medicine's Key Specialty Cooperative Group for Orthopedics and Traumatology. The achievements have been promoted and applied nationwide among member units, benefiting more than 28,285 patients and improving the diagnosis and treatment level of related diseases across the country.
