Currently, the incidence and mortality rates of diabetes in China rank first globally, posing a serious threat to human health. Under the guidance of the academic thought 'Treating the Spleen to Stabilize the Five Zang Organs,' our research team has inherited Professor Cai Gan, a renowned national traditional Chinese medicine practitioner, clinical experience. Through long-term clinical practice, we have formulated a series of prescriptions based on the basic principle of 'Strengthening the Spleen' to prevent and treat diabetes and its complications in stages, achieving satisfactory clinical results. The innovative achievements are as follows: 1. Based on the treatment principle of 'Strengthening the Spleen, Clearing Heat, and Transforming Dampness,' we have developed a prescription called Strengthening the Spleen and Clearing Decoction. On the basis of lifestyle intervention, using this prescription for a cumulative total of 5,885 patients with prediabetes, multiple randomized double-blind placebo-controlled studies have confirmed that: Compared with the conventional control group, the annual conversion rate of diabetes was reduced by 60.2%. It also significantly improved the clinical manifestations of 'Qi and Yin deficiency (spleen deficiency)' in high-risk diabetic populations, enhanced the prevention and treatment level of such diseases, and established a standardized clinical protocol. 2. With the principle of 'strengthening the spleen and clearing heat', the Spleen Strengthening and Heat Clearing Formula and its component formula, Qinghua Granules, have been used to treat 15,945 cases of diabetes. Multiple randomized double-blind placebo-controlled studies have confirmed an efficacy rate of 83.5%. At the same time, using continuous blood glucose monitoring for 72 hours as the assessment method, it has been proven that the Spleen Strengthening and Heat Clearing Formula can reduce the mean amplitude of glycemic excursions (MAGE) in diabetic patients by 50.9%, The mean absolute difference in daytime blood glucose (MODD) decreased by 65.8%, effectively reducing fluctuations in both daytime and intra-day blood glucose, decreasing the risk of hypoglycemia, and effectively maintaining blood glucose homeostasis. 3. Following the treatment principle of 'tonifying the spleen and activating blood circulation', the DM120 formula was formulated, which has been used to treat a total of 2500 cases of early diabetic nephropathy. Multiple randomized, double-blind, placebo-controlled studies have confirmed that the effective rate reached 78.8%, with a decrease in creatinine (Cr) by 60.5%, urea nitrogen (BUN) by 35.8%, and urinary albumin excretion rate (UAER) by 68.8%. Moreover, it significantly improved the clinical symptoms of patients with traditional Chinese medicine (TCM), effectively preventing and treating diabetic nephropathy. 4. Following the treatment principle of 'tonifying the spleen and promoting meridian flow', the Lingyi Capsules ( The clinical scheme of combined traditional Chinese medicine (TCM) for spleen strengthening and clearing heat, known as 'Zhi Jian Qing Hua Fang', has been used to treat 1800 cases of diabetic peripheral neuropathy. Multiple randomized double-blind placebo-controlled studies have confirmed that the effective rate is 72.6%. Through neurophysiological testing, it was found that the patient's pain grading index and existing mild pain have significantly decreased (P<0.01), which has significantly improved the clinical symptoms of lower limb numbness and pain in patients, effectively preventing and treating diabetic peripheral neuropathy. 5. Revealing new mechanisms of action for spleen-strengthening herbal medicines. The key target for prevention and treatment of prediabetes with 'Zhi Jian Qing Hua Fang' is glucagon-like peptide-1 (GLP-1); the key target for prevention and treatment of diabetes with 'Qing Hua Granules' is the bitter taste receptor; the key target for prevention and treatment of diabetic nephropathy with DM120 is TFG-β1; The key target of the spiritual capsule in preventing and treating diabetic peripheral neuropathy is Na+-K+ATPase. The research results have been applied in multiple communities in Shanghai as part of a comprehensive traditional Chinese medicine prevention and treatment plan for high-risk groups of diabetes intervention and the treatment of type 2 diabetes and its complications in over 50 hospitals and community health service centers in Shanghai.
