A large sample epidemiological survey of 2,771 cases found that the key pathogenesis of chronic kidney disease is 'stasis and damp-heat'. Focusing on various pathogenic factors of chronic kidney disease, data mining and analysis of TCM syndromes and clinical materials were collected from 520 IgA nephropathy patients, 477 CKD1-4 stage patients, 939 CKD2-5 stage patients, and 835 community hematuria patients. It was proposed that 'stasis and damp-heat' is the core pathogenesis of exacerbating CKD progression, which runs through the entire course of the disease. An effective TCM syndrome differentiation optimization scheme and series of prescriptions were established: Kangxianling formula, Jianpi Qinghua formula, Renshu Chongji, Sican formula, and Guben Tongluo formula. A prospective clinical study of 148 cases of severe IgA nephropathy with liver and kidney yin deficiency type, using an optimized TCM syndrome differentiation scheme, Prednisone and Cosyntropin were used as the control groups, observed for 6 months, showing that the TCM syndrome differentiation group could significantly improve clinical symptoms of TCM, reduce proteinuria, increase GFR, lower IgA nephropathy risk index, with a total effective rate of 71.05%, compared to the control group with a total effective rate of 38.89%. A follow-up study on IgA nephropathy lasting ten years found that simple microscopic hematuria type IgA nephropathy does not necessarily have a good prognosis, and the appearance of hypertension or interstitial kidney disease indicates a poor prognosis. A study on 396 cases of chronic kidney disease (CKD1-2 stage) with proteinuria TCM optimization treatment plan validation and promotion application research, and a clinical prospective double-blind study on 270 cases of spleen deficiency and damp-heat type chronic kidney disease (CKD3 stage), observed for 6 months, the results showed that TCM syndrome differentiation treatment group and spleen strengthening and clearing heat formula significantly improved TCM symptoms, reduced proteinuria, and improved renal function. Downregulate NF-κB and PI3K-Akt-mTOR signaling pathways to inhibit renal fibrosis in Smad2/3 gene knockout mice; 2. Suppress the excessive activation of interleukins and inflammatory factors in FSGS rats' kidneys, regulate the CD4+/CD8+ ratio and cellular immune factors, thereby inhibiting immunologically mediated inflammatory damage leading to renal fibrosis, etc. Establish an integrated research platform for clinical and scientific research, forming a model of traditional Chinese medicine research that promotes the transformation of scientific research into practice and theoretical innovation based on clinical data. Lead the development of TCM nephrology discipline. Take the lead in formulating industry standards for the differentiation and typing of chronic kidney disease as well as efficacy evaluation, develop 2 nephrology diagnosis and treatment guidelines, and 2 TCM clinical pathways. The TCM diagnosis and treatment plan for chronic kidney disease has been promoted in more than 80 units nationwide, benefiting patients from about 20 countries and regions at home and abroad.
