This project belongs to the field of clinical and basic research in traditional Chinese medicine (TCM) and integrated traditional Chinese and Western medicine. In response to the medical challenge of high recurrence and metastasis rates after colorectal cancer surgery, over the past 15 years, the research team has combined clinical and basic research by focusing on the dynamic evolution of pathogenesis, syndrome patterns, and modern biological foundations to study the prevention and treatment of colorectal cancer recurrence and metastasis with TCM. (1) The research team proposed and demonstrated a new model for the pathogenesis-syndrome pattern-treatment method in colorectal cancer. They studied 647 cases of colorectal cancer before and after surgery, before and after recurrence and metastasis, etc., confirming that spleen deficiency with damp-heat is the key pathogenesis of colorectal cancer, discovering that damp-heat is the pathological basis for chemotherapy resistance in colorectal cancer, and identifying spleen deficiency as the main factor in the invasion and metastasis of colorectal cancer. They proposed that strengthening the spleen and detoxifying are the basic treatment methods for preventing and treating colorectal cancer with TCM. (2) The team established a syndrome differentiation and treatment plan based on the basic principle of strengthening the spleen and detoxifying, which reduced the recurrence and metastasis rates after colorectal cancer surgery. Extended survival period. Based on the basic pathogenesis characteristics of colorectal cancer recurrence and metastasis, a multi-center clinical study combined disease, syndrome, prescription, and efficacy found that the spleen-strengthening and detoxifying prescription reduced chemotherapy resistance, inhibited the recurrence and metastasis rate of colorectal cancer after surgery, and extended the median survival period of late-stage colorectal cancer from 10.2 months to 13.6 months, according to domestic leading levels. It was discovered that the classical prescription Zuojin Pill reduces chemotherapy resistance and inhibits the invasion and metastasis of colorectal cancer. (3) Revealed the biological basis of spleen deficiency and damp-heat syndrome, the international first report of COX-2-activated signaling pathways in the regulation mechanism of colorectal cancer chemotherapy resistance and recurrence metastasis, providing new targets for the integrated treatment of tumors with traditional Chinese and Western medicine. It was found that the COX-2-JNK/c-Jun--MDR1 signaling pathway regulates the P-gp mediated multidrug resistance mechanism of colorectal cancer. Explain the biological basis of multi-drug resistance in colorectal cancer mediated by damp-heat syndrome; discovered that COX-2 activation of GSK-3β/β-catenin, VEGF/MEK/ERK signaling networks promotes colorectal cancer invasion, revealing the biological basis of colorectal cancer metastasis mediated by spleen deficiency syndrome. Propose a traditional Chinese medicine theory that spleen deficiency and damp-heat cause colorectal cancer from chemotherapy multi-drug resistance to metastasis. Construct a biological information regulatory network targeting COX-2 for colorectal cancer with spleen deficiency and damp-heat syndrome. Reveal the evolution law of colorectal cancer from resistance to metastasis, 'spleen deficiency and damp-heat' is the pathological basis of resistance to metastasis, during resistance, the damp-heat syndrome type is more prominent, as colorectal cancer progresses, the phenotype of spleen deficiency gradually becomes prominent, spleen deficiency and damp-heat are the basic pathogenesis characteristics of colorectal cancer metastasis. The mechanism of action of Jianpi Jiedu Formula in inhibiting colorectal cancer metastasis has been revealed. The regulatory targets of Jianpi Jiedu Formula on genes related to colorectal cancer invasion and metastasis have been identified, as well as the main components resveratrol and tanshinone IIA, which inhibit colorectal cancer invasion and metastasis through signal pathways such as MALAT1/β-catenin, COX-2-mediated JNK/c-Jun, etc., providing new evidence for the prevention and treatment of colorectal cancer invasion and metastasis with resveratrol and tanshinone IIA. (5) A model biology research platform with traditional Chinese medicine characteristics has been established. Mouse models with combined syndromes such as damp-heat syndrome, spleen deficiency syndrome, kidney deficiency syndrome in colorectal cancer have been developed, along with a cell drug screening model for reporting genes; a traditional Chinese medicine nano-gene delivery system capable of actively recognizing invasive and metastatic genes has been established to enhance the efficacy of traditional Chinese medicine in inhibiting colorectal cancer metastasis. Application Promotion: Published 122 papers, including 25 in SCI journals with an impact factor of 76.382, and cited 509 times. Applied for 20 national invention patents, of which 4 were authorized.
