I. Background: According to the 2002 National Nutrition and Health Survey data, the prevalence of hypertension among adults in China is 1.8%, with an estimated 160 million hypertensive patients. Hypertension ranks first among the risk factors for total mortality in China. In 2003, direct medical expenses amounted to 30 billion yuan. The increasing prevalence and heavy socio-economic burden have made China one of the countries most severely affected by hypertension. Blood pressure variability is a hot topic in the latest research on the prevention and treatment of hypertension. Traditional Chinese medicine (TCM) has a long history in the prevention and treatment of hypertension. Combining modern medical progress to study TCM theories on hypertension and thereby improving the efficacy of TCM in preventing and treating hypertension is one of the research directions of this project group. As early as 2004, this project group reported the relationship between TCM syndrome differentiation and insulin resistance and lipid metabolism in patients with hypertension (see the attachment for details). II. Main Research Content: This study observes different populations ( The distribution patterns of TCM syndrome types in hypertensive patients from different populations (general population, perimenopausal hypertension group), regions (Shanghai, Zhejiang, Xinjiang), and ethnicities (Uyghur, Han) were observed and evaluated. In different populations, regions, and ethnicities, the correlation between TCM syndrome types and ambulatory blood pressure (including: 24-hour, daytime, nighttime mean blood pressure values, 24-hour, daytime, nighttime blood pressure load values, pulse pressure difference, and blood pressure circadian rhythm) was observed and evaluated; the correlation between TCM syndrome types and blood pressure variability; the correlation between TCM syndrome types and target organ damage (carotid artery atherosclerosis, left ventricular hypertrophy, microalbuminuria, etc.). Based on the study of syndrome types, corresponding treatment with formulas (warming yang method for treating yang deficiency type hypertension, nourishing yin and suppressing yang method for treating yin deficiency and hyperactive yang type hypertension) was applied to treat hypertension. The effects were assessed using various indicators including: highest blood pressure over 7 consecutive days; morning blood pressure, ambulatory blood pressure, blood pressure variability, hypertension target organ damage, Traditional Chinese medicine syndromes and sex hormones are used as observation indicators to evaluate clinical efficacy from multiple perspectives and levels (see attachment for details). III. Key Innovations 1. The study elucidated the distribution patterns of TCM syndrome types among different populations, regions, and ethnic groups with hypertension. It was proposed that there is a correlation between TCM syndrome types and ambulatory blood pressure; there is a correlation between TCM syndrome types and blood pressure variability; there is a correlation between TCM syndrome types and target organ damage. Based on the research on syndrome types, prescription-syndrome correspondence (treatment of Yang deficiency type hypertension with warming yang method, treatment of Yin deficiency and Yang hyperactivity type hypertension with nourishing yin and suppressing yang method) has significantly improved patients' continuous 7-day highest blood pressure, morning blood pressure, ambulatory blood pressure, blood pressure variability, hypertensive target organ damage, and TCM syndromes, showing significant therapeutic effects. Treatment of perimenopausal hypertension patients with nourishing yin and suppressing yang method not only improves blood pressure, It can also improve TCM syndromes and enhance ovarian function. Patents: One new patent application has been filed, 'A traditional Chinese medicine combination for treating renal hypertension and its preparation method and use,' with the application number: 201410729557.9. (For details, please refer to the attachment) V. Papers and Promotion Application Published papers totaling 11, cited 73 times. Additionally, one paper is under acceptance. One conference paper was presented. Three graduate students were trained. Three national and municipal-level study classes were held. The research results have been widely applied in our hospital and promoted at Shanghai Longhua Hospital, Yueyang Integrated Traditional Chinese and Western Medicine Hospital, Traditional Chinese Medicine Hospital, Putuo District Central Hospital, Huaihai Middle Road Street Community Health Service Center, and Xinjiang Medical University Affiliated Traditional Chinese Medicine Hospital. The research results are of great significance for the construction and formation of a new framework for TCM syndrome differentiation and classification system in hypertension, as well as for developing new clinical diagnosis and treatment standards for hypertension. They also have significant economic and social benefits, with a broad market prospect.
