I. Main Technical Content This project belongs to the clinical basic research of traditional Chinese medicine internal medicine. In response to issues related to dyskinesia in Parkinson's disease (LID), based on the principle of syndrome-pattern correspondence, research has been conducted for over a decade from literature mining, animal experiments, and clinical applications, resulting in the following innovative achievements: 1. Established the TCM syndrome diagnostic criteria and literature database for LID. Following the principles of evidence-based medicine, we organized ancient and modern literature, combined clinical retrospective studies, and applied data mining technology to sort out common syndromes, treatment methods, and prescriptions, establishing the TCM syndrome diagnostic criteria, filling a gap in such research. 2. First proposed the concept of 'toxicity' as the pathogenesis of LID. Based on literature research and combined with clinical experience, it was suggested that the occurrence and development of LID are closely related to 'toxicity'. The pathogenesis includes deficiency, stasis, phlegm, wind, Toxic mutualism causing illness should be treated primarily with 'nourishing yin and extinguishing wind, supplemented by promoting blood circulation to remove stasis, detoxifying and dispersing nodules'. This approach has received widespread attention from domestic peers, with related papers being cited multiple times and the author invited to give reports at academic conferences on several occasions. 3. The first LID rat model of Yin deficiency with dynamic wind syndrome was established, and its material basis was explored. A unilateral injection of 6-hydroxydopamine at two points in the brain was used to damage the substantia nigra, followed by further intraperitoneal injections of levodopa/carbidopa for two weeks. By analyzing symptoms, signs, behavior, tongue appearance, relevant objective indicators, drug counterarguments, etc., it was confirmed that this model represents a rat model of Yin deficiency with dynamic wind syndrome (LID). Material basis research showed that at different time points of 2, 4, and 6 weeks, the model rats exhibited behavioral changes, oxidative stress, excitatory amino acids, apoptosis, dopamine, dopamine D1 receptor, dopamine D2 receptor, proenkephalin, preproenkephalin, There are significant changes in multiple aspects such as DAT, TH, gene expression, GDNF, etc. This achievement provides a good carrier for the future integrated research of LID (Liver Yin Deficiency with Wind Agitation Syndrome) between traditional Chinese and Western medicine, and also lays the foundation for the development of clinical treatment drugs. 4. Established the basic prescription for treating the disease - Rehmannia Decoction. Based on the literature database and combined with the etiology and pathogenesis of LID, the basic prescription for treating the disease - Rehmannia Decoction was established. The successful development of this prescription provides ideas and basis for the optimized treatment of LID and the research and development of innovative drugs. 5. Promoted the application of Rehmannia Decoction in clinical practice. Clinical application shows that Rehmannia Decoction can significantly improve patients' motor function, delay the progression of the disease, extend the 'on' period time for those with 'switching' phenomena, and reduce the duration of the 'off' period. Currently, it has been supported by Shanghai's science and technology support project in the field of traditional Chinese medicine for new drug research and development. II. Characteristics 1. Established the first TCM syndrome diagnosis criteria for LID. 2. Proposed the concept of 'toxin' as the pathogenesis of LID for the first time. 3. Established the rat model of LID with Yin deficiency stirring wind syndrome for the first time and explored its material basis. 4. Created the basic prescription for treating this disease - Rehmannia Decoction. 5. Promoted and applied Rehmannia Decoction in clinical practice. III. Application and Promotion During the project research period, a total of 83 papers were published publicly, including 3 SCI-indexed papers, 2 EI-indexed papers, and the papers were cited 338 times; one patent was authorized; participated in international conferences 7 times, domestic conferences 16 times; relevant academic views were cited by the national 'Eleventh Five-Year' planning textbook for higher education graduate students 'Acquisition and Utilization of Traditional Chinese Medicine Literature Information' (compiled by Jiang Yongguang, People's Health Publishing House, January 2009), and received high praise from peers; 18 graduate students have been trained; one master's thesis was awarded the 2010 Shanghai Excellent Graduate Thesis; clinical trials in five tertiary-level hospitals were recognized by peers and widely praised by patients; through long-term research and accumulation, young scientific researchers have been cultivated, stable research directions have been formed, a new academic growth point has emerged, and the development of the discipline has been promoted.
